About the Role:

This is a part-time position available at a leading healthcare facility, supporting patients and families through complex journeys of care. You will work as part of a multidisciplinary team, collaborating with other healthcare professionals to deliver exceptional patient outcomes.

Selection Criteria:
  • Demonstrated expertise in Renal and Palliative Care Social Work, with a strong focus on clinical experience
  • Knowledge and experience in advanced care planning, psychosocial aspects of caring for dying patients, grief counseling, and community support
  • Excellent communication, negotiation, decision-making, and advocacy skills
