Renal Care Specialist

Social workers in this role provide critical support to patients with renal disease and their families, promoting holistic care and improved patient outcomes.
Key Responsibilities:- Deliver psychosocial counseling services to patients and families affected by renal disease
- Facilitate advance care planning and informed decision-making processes
- Collaborate with healthcare professionals to ensure comprehensive patient care
- Educate teams and mentor students on best practices in renal care
This is a part-time position available at a leading healthcare facility, supporting patients and families through complex journeys of care. You will work as part of a multidisciplinary team, collaborating with other healthcare professionals to deliver exceptional patient outcomes.
Selection Criteria:- Demonstrated expertise in Renal and Palliative Care Social Work, with a strong focus on clinical experience
- Knowledge and experience in advanced care planning, psychosocial aspects of caring for dying patients, grief counseling, and community support
- Excellent communication, negotiation, decision-making, and advocacy skills
