About the Role

We are seeking a highly skilled and motivated Clinical Pharmacist to join our team at Forster Private Hospital in NSW. As a key member of our pharmacy team, you will be responsible for providing clinical pharmacy services to inpatients and assisting in our dispensary to ensure that all pharmacy activities are completed to a high standard.

Key Responsibilities
  • Conduct medication chart reviews and medication reconciliation to ensure accurate and safe medication use.
  • Dispense and supply medication and pharmacy products to patients and healthcare professionals.
  • Coordinate patient medication requirements for inpatient stays, day infusions, and discharge to ensure seamless care.
  • Develop and maintain professional relationships with key stakeholders to provide excellent customer service to clients and patients.
  • Support patients, carers, hospital staff, and associated employees to ensure their medication needs are met.
  • Educate and advise staff and customers on health promotion, disease prevention, drug selection, and the proper use of medicines.
  • Review and complete monthly reports to track pharmacy performance and identify areas for improvement.

Requirements
  • You must be a current AHPRA registered pharmacist.
  • You should have a strong knowledge of the Pharmaceutical Benefits Scheme (PBS).
