About the Role

We are seeking a highly skilled and experienced Pharmacist to join our team at Icon Group in a permanent part-time position. As a Hospital Pharmacist, you will be responsible for delivering exceptional pharmacy services to patients in a specialized hospital setting.

Key Responsibilities
  • Conduct medication chart reviews and medication reconciliation to ensure accurate and safe medication management.
  • Dispense and supply medication and pharmacy products to patients, adhering to strict quality and safety standards.
  • Coordinate patient medication requirements for inpatient stays, day infusions, and discharge, ensuring seamless transitions and optimal patient outcomes.
  • Develop and maintain professional relationships with key stakeholders, including hospital staff, patients, and carers, to provide excellent customer service and support.
  • Support patients, carers, and hospital staff in understanding medication regimens, health promotion, and disease prevention.
  • Review and complete monthly reports to track medication usage, patient outcomes, and quality improvement initiatives.
About You

  • Be a current AHPRA registered pharmacist with previous experience in an Australian or New Zealand based pharmacy.
