About the Role

We are seeking a highly skilled and experienced Pharmacist to join our team at Slade Pharmacy in a Hospital Pharmacist role.

Key Responsibilities
  • Conduct thorough medication chart reviews and medication reconciliation to ensure patient safety and optimal treatment outcomes.
  • Dispense and supply medications and pharmacy products in a timely and accurate manner.
  • Manage patient medication needs during inpatient stays, day infusions, and discharges, providing expert advice and guidance as needed.
  • Build and maintain strong relationships with healthcare professionals and stakeholders to ensure seamless patient care and service delivery.
  • Provide education and support to patients, carers, and hospital staff on medication management, health promotion, and disease prevention.
  • Stay up-to-date with the latest clinical guidelines and research, applying this knowledge to improve patient outcomes and service delivery.
Requirements
  • AHPRA registration as a pharmacist.
  • Relevant experience working in an Australian pharmacy setting, preferably in a hospital or clinical environment.
  • Strong communication and interpersonal skills, with the ability to work effectively in a team and with diverse stakeholders.
  • Attention to detail, self-motivation, and a passion for continuous learning and professional development.
