This role focuses on promoting family health and wellbeing by working with children, young people, elders and families. It involves creating and delivering responses to population health needs in collaboration with the local community.
- Key Responsibilities:
- Develop and implement programs that improve health outcomes for children, families, and communities under the guidance of a comprehensive framework.
- Work closely with children and families during early learning sessions, including home visits and other outreach strategies, prioritizing optimal health promotion.
- Collaborate with First Nations staff and family members to provide culturally appropriate health information.
- Support First Nations families in valuing their ideas, strategies, and aspirations.
- Maintain regular communication with local First Nations people and elders to inform the direction of this role.
- Liaise with community members to ensure active promotion of local cultural knowledge and practice.
- Program Development and Implementation
- Provide health promotion support at individual, family, and community levels.
- Design and deliver health promotion activities across various settings, both on-site and within the community.
- Contribute to resource development, including educational materials, videos, and books, in collaboration with or guided by First Nations staff and community members.
- Offer advice and recommendations on health promotion needs in the community.
- Develop personalized health plans with families.
- Offer support for individuals and families, including assistance with health appointments and travel arrangements.
- Data Management and Reporting
- Ensure accurate recording of user data to meet organizational monitoring and evaluation requirements.
- Guarantee compliance with standards and maintain confidentiality and security of consumer information.
- Manage organized and up-to-date files related to individuals.
- Be an engaged and innovative member of the team.
- Model healthy behaviors and promote them among others.
- Contribute to a strong, respectful, and supportive work environment.
- Participate in professional development opportunities.
- Follow established policies and procedures.
- Foster positive working relationships across teams.
- Represent the organization in meetings as needed.
- Undertake tasks as required to support regional operations.
