About the Role

We are seeking a compassionate and dedicated Care Coordinator to join our team at Uniting Aged Care. As a Care Coordinator, you will play a vital role in supporting clients who are transitioning out of the hospital and into our Transitional Aged Care Program.

Key Responsibilities

  • Provide practical support and companionship to clients in their homes and communities.
  • Assist with daily living tasks, such as housekeeping, grocery shopping, and meal preparation.
  • Support clients with personal care, including bathing, dressing, and medication administration.
  • Transport clients to appointments and on shopping trips.
  • Accompany clients on outings and social support activities.

Requirements

  • Certificate III in Aged Care, Individual Support or equivalent.
  • Experience in providing client-centered care.
  • Current First Aid certificate.
  • Valid driver's license and access to a comprehensively insured vehicle.

Mandatory Vaccinations

  • Diphtheria, Tetanus & Pertussis.
  • Hepatitis B.
  • Measles, Mumps & Rubella (MMR).
  • Varicella.
  • Influenza.
  • Tuberculosis.
  • Covid-19.
