Nurse/Midwife Clinical Specialist Preventable Chronic Conditions

This role is an opportunity to make a meaningful difference in the lives of people living in remote NT communities.

The successful candidate will be responsible for providing specialist clinical outreach care to clients and specialist support, advice, and education to PPHC staff with a focus on improving outcomes in early detection and management of preventable chronic conditions in a culturally sensitive manner.

You'll be accountable for providing sound professional advice and assessment, planning, review, evaluation, and coordination of comprehensive client care and/or case management.

Required Skills and Qualifications
  • Current registration with Nursing and Midwifery Board of Australia or eligible for same.
  • High-level clinical knowledge and skills in Preventable Chronic Conditions or related field, supported by strong decision-making and problem-solving skills.
  • Commitment to a learning culture through clinical guidance and mentoring.
  • Ability to travel to remote communities by 4WD or light aircraft as required.
  • Hold a current NT driver's licence (or eligible to obtain) and be confident driving a manual vehicle in regional and remote settings.
