Holistic Chronic Disease Management Specialist

As a Care Coordinator, you will play a vital role in supporting Aboriginal people with chronic health conditions living in remote areas. You will work collaboratively with clients, general practitioners, and other health service providers to provide care services that optimise health outcomes.
The position requires the ability to travel to Alice Springs or interstate for specialist appointments, as well as occasional remote travel. To be successful in this role, you will require:
- Registered or eligible to register with AHPRA as a registered nurse or Aboriginal Health Practitioner with at least 2 years clinical experience in chronic disease management and/or primary health care
- Excellent understanding of primary health care and sound clinical knowledge of chronic disease management
- Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control
To excel in this role, you will need:
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Capacity to understand and respect the cultural needs of Aboriginal clients
