Dementia Support Specialist

About the Role
We are seeking a Dementia Support Specialist to join our team at HammondCare in Hobart. As a key member of our team, you will provide on-the-ground support and implement relationship-based support strategies to enhance the lives of individuals living with dementia.
Key Responsibilities
- Work collaboratively with peers to develop the skills and expertise of aged care staff and carers to understand the needs of individuals living with dementia.
- Support the implementation of recommendation plans on the ground, using modelling and coaching techniques, and seek guidance from Team Leader and Senior Consultant as required.
- Facilitate and support the trial and implementation of brokerage items with clients and their carers, and seek guidance from Team Leader and Senior Consultant as required.
Requirements
- Strong organisational abilities, including effective time management and prioritisation in a busy work environment.
- The ability to work both independently and as part of a team.
- Demonstrated commitment to excellence in customer service principles, including conflict resolution, to ensure exceptional service to internal and external customers.
- A current driver's licence and safe driving record.
- Willingness to travel and work intrastate and interstate for up to five days at a time to support the delivery of services.
- Willingness to work flexible hours as required to meet the assessed needs of clients and carers.
- Qualification in a relevant field, such as Allied Health assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health, or similar.
