About the Role

This role provides spiritual care, leadership, and support to clients, their families, team members, and pastoral care volunteers. The Pastoral Care Coordinator will visit clients in their homes across the South Coast, requiring access to a car and a valid driver's license.

Key Responsibilities
  • Assessing clients' spiritual and emotional needs and developing a care plan to ensure their needs are acknowledged and documented.
  • Providing spiritual and emotional support to clients and their families.
  • Liaising with religious leaders to coordinate spiritual support for clients and their families.
  • Training team members in pastoral care, including boundaries, grief, and loss, and other areas as identified.
  • Working with team members to extend their skills in meeting clients' spiritual needs.
  • Participating in staff meetings, in-services, and other events to promote pastoral care opportunities and partnerships.
About You
  • A commitment to HammondCare's Christian Mission and values.
  • Strong understanding and experience of people living with life-limiting illnesses, including dementia, and their carers.
  • Strong personal and active Christian faith and church involvement.
  • Theological or pastoral care qualifications and experience.
  • Demonstrated capacity to care for and enhance the wellbeing of those in need.
  • Highly developed communication skills, including listening, compassion, and empathy.
  • Experience leading and supporting a team.
  • Strong administrative and computer skills.
