We are seeking an experienced and compassionate health professional to take on this newly created position of Complex Discharge Coordinator. This role plays a pivotal part in improving patient outcomes and care transitions.


This is a key leadership role enhancing patient flow, reducing hospital length of stay and preventing avoidable readmissions. It also supports system-level improvements through the design and implementation of innovative care pathways.

  • The successful candidate will play an essential role in facilitating safe, timely, and coordinated discharges for patients with complex care needs.
  • In close collaboration with multidisciplinary teams, patients, families, and external service providers, they will ensure continuity of care beyond the hospital setting.
  • The ideal candidate will have strong communication and interpersonal skills to collaborate effectively and proven problem-solving skills with a proactive approach to overcoming barriers to safe discharge.

Required Skills and Qualifications">

The successful candidate must possess:

  • Registered Nurse, Allied Health Professional, or relevant health qualification with current registration (if applicable).
  • Minimum 5 years' clinical experience.
  • Experience and knowledge in discharge planning, care coordination or case management within hospital or community health settings.
