Role Overview

This role offers an exciting opportunity to support older people in regaining independence and confidence at home. Our Transitional Care Program provides short-term, goal-oriented care for up to 12 weeks after hospital, helping clients optimise their functioning and independence.

About the Position

  • Work as part of a multidisciplinary team to assess, plan, and coordinate tailored services.
  • Support older people to optimise their independence and quality of life in their own homes.
  • Apply your knowledge, organisational skills, and problem-solving in a role that blends care, coordination, and empowerment.

Key Responsibilities

  • Assess clients' needs and develop individualised plans to promote independence and well-being.
  • Coordinate with healthcare professionals and community services to ensure seamless care delivery.
  • Empower clients to take control of their health and wellbeing through education and support.

Requirements

  • Significant experience in a health, aged or community service environment with an understanding of home care packages, CHSP guidelines and Aged Care Quality Standards.
  • Excellent written, verbal and interpersonal communication skills with the ability to inclusively communicate with a diverse range of people.
  • Excellent customer service focus and strong person-centred skills with the ability to effectively engage clients requiring services.
  • Demonstrated ability to organise and prioritise tasks, work effectively under pressure, problem solve and meet deadlines.
  • Current Western Australian 'C' Class driver's license.
  • National Police Certificate (obtained within the last 12 months).
