Job Summary:

We are seeking a highly skilled and experienced Care Manager to join our team at Bupa Banora Point. As a Care Manager, you will play a key role in leading and managing the delivery of person-centred care to our residents, ensuring the highest standard of ongoing assessment, care planning, and evaluation.

About the Role:

  • Oversee the daily clinical operations of the care home, ensuring the highest standard of care is delivered to our residents.
  • Support and lead the care home clinical team, including coaching, mentoring, staff allocations, and delegation.
  • Review clinical incidents, complaints, and changes, ensuring compliance with regulatory requirements.
  • Lead compliance with AN ACC assessments, ensuring accurate and timely completion.
  • Manage person-centred care planning and evaluation for residents, including case conferencing with families and carers.
  • Prepare and conduct clinical governance audits, ensuring the highest standard of care is maintained.
  • Liaise with allied health, medical practitioners, and other services to provide optimal resident care.

Requirements:

  • Bachelor of Nursing with current AHPRA registration is essential.
  • Post-graduate qualification in aged care is highly regarded.
  • Experience managing a minimum of 30 beds within the aged care industry is required.
  • A proven track record in delivering quality clinical care within an aged care or acute setting is essential.
  • Demonstrated clinical assessment skills and care planning capabilities are required.
  • Highly effective communication, leadership, and relationship management skills are essential.
  • An understanding of Resident Rights, Aged Care Accreditation Standards, and Outcomes is required.
