About the Role

We are seeking a highly skilled and experienced Care Manager to join our team at Bupaoptical. As a Care Manager, you will play a critical role in ensuring the highest standard of ongoing assessment, care planning, and evaluation for our residents.

Key Responsibilities
  • Oversee the daily clinical operations of our care home, ensuring seamless delivery of person-centred care.
  • Support and lead the care home clinical team, providing coaching, mentoring, and performance management.
  • Review clinical incidents, complaints, and changes, ensuring compliance with regulatory requirements.
  • Lead compliance with AN ACC assessments, ensuring accurate and timely completion.
  • Manage person-centred care planning and evaluation for residents, including case conferencing with families and carers.
  • Prepare and conduct clinical governance audits, ensuring the highest standards of care.
  • Liaise with allied health, medical practitioners, and other services to provide optimal resident care.
Requirements
  • Bachelor of Nursing with current AHPRA registration.
  • Post-graduate qualification in aged care highly regarded.
  • Experience managing a minimum of 30 beds within the aged care industry.
  • A proven track record in delivering quality clinical care within an aged care or acute setting.
  • Demonstrated clinical assessment skills and care planning capabilities.
  • Highly effective communication, leadership, and relationship management skills.
  • An understanding of Resident Rights, Aged Care Accreditation Standards, and Outcomes.
