**About the Role:**

We are seeking a dedicated Allied Health Assistant to join our multidisciplinary team in At Home Subacute. In this role, you will have the opportunity to work collaboratively with Physiotherapists, Occupational Therapists, Speech Pathologists, Social Workers, Psychologists and Dietitians to provide comprehensive care to clients in their homes.

**Key Responsibilities:**

  • Assist with client assessments, goal setting and progress monitoring to support clients in achieving their goals.
  • Work collaboratively with a multidisciplinary team to develop and implement treatment plans.
  • Provide education and support to clients and their families on health-related issues.

**Requirements:**

  • Certificate 4 in Allied Health Assistant qualification.
  • Demonstrated experience as an Allied Health Assistant.
  • Current Victorian Drivers Licence.
  • Capacity to work independently and autonomously in the community.
