Community Care Coordinator Lead

We are seeking an experienced Senior Community Care Coordinator to join our team in Regional Queensland.

About the RoleThe successful candidate will be responsible for collaborating with clients, their families, and healthcare providers to create personalized care plans that focus on chronic disease management and well-being.
- Developing and implementing comprehensive care plans that meet the needs of clients
- Collaborating with multidisciplinary teams to ensure seamless delivery of care services
- Providing exceptional communication and advocacy skills to support clients and their families
To be successful in this role, you will require:
- A relevant Primary or Allied Health qualification (Registered Nurse preferred)
- Proficiency in systems such as E Tools, My Aged Care, DEX Data Exchange, GeoOp, Medicare, & Proda
- A strong foundation in care coordination and experience with community aged care services
- Thorough knowledge of Aged Care and NDIS legislation
- Leadership skills that inspire teams, paired with outstanding communication and collaboration abilities
- Experience in working with complex health scenarios and managing multi-disciplinary teams
- Ability to juggle multiple priorities with ease
- Current Disability Services Positive Notice Card or Working with Children Check(or ability to obtain)
- Valid Queensland Class C Driver's Licence
- National Criminal History Check and up-to-date vaccinations (or ability to obtain)
