Job Description:

We are seeking a dedicated and experienced professional to oversee client care and ensure seamless service delivery.


Main Responsibilities:
  • Plan, manage, coordinate, and review client goals to deliver high-quality care services.
  • Communicate effectively with team members and clients to develop personalized care plans tailored to their unique needs and preferences.
  • Establish agreements with clients and their families regarding care services and associated costs.
  • Maintain accurate records of interactions with clients, family members, or other stakeholders, sharing relevant information with appropriate team members.

Required Qualifications and Skills:

To excel in this role, you will need:

  • A degree in a health-related field (social work, allied health, or related discipline).
  • Relevant experience working with seniors or aged care.
  • A minimum of 1 year of experience in a similar role, demonstrating efficiency, organization, and strong communication skills.
