About Our Program

Our team delivers person-centred services to individuals and families at risk of homelessness. We provide holistic case management, accommodation, outreach support, financial assistance, connection to specialist services, and help clients achieve housing stability, social inclusion, and stronger connections with community supports.

This role is critical in delivering a high-quality service that sets a standard of care for those we support, our partners, and the wider community. As an Outreach Case Manager, you will contribute to our nationwide Homelessness Stream Model of Care by providing tailored support to adults and families experiencing or at risk of homelessness, including accompanying children.

Key aspects of this role include:

  • Delivering case management services to clients, ensuring their needs are addressed and they receive the necessary support.
  • Working collaboratively with other teams and organisations to provide comprehensive support and services.
  • Developing and maintaining relationships with community services and agencies to ensure seamless referrals and transitions.
