Role Overview:

The Pathway Coordinator plays a pivotal role in supporting individuals with disabilities and their families.

Main Responsibilities:
  • Support Navigation: Provide expert guidance to customers ensuring seamless navigation of the National Disability Insurance Scheme (NDIS) Access Pathway and Participant plans.
  • Document Assistance: Assist clients in completing and submitting required documents for Scheme Access Requests.
  • Community Referrals: Refer individuals to Local Area Coordinators or Early Childhood Teams as needed, following documentation verification.
  • Asset Mapping: Develop and maintain asset maps establishing community connections for individuals aged 9 and above.
  • Personalized Plans: Build personalized Community Connections Plans as required.
  • Collaboration: Collaborate with multi-disciplinary teams to ensure efficient management of the Early Childhood pathway for children under 9.
  • Towards Inclusion Framework: Utilize the Towards Inclusion Framework (TIF) to enhance individual and family capacity, promoting choice, control, and community participation.
  • Information Provision: Ensure timely information provision and linkages to mainstream services for participants and non-participants.
Workplace Health & Safety:
  • Hazard Reporting: Report workplace hazards, injuries, and incidents, actively participating in WHS improvement activities.
  • Personal Safety: Protect personal safety and the safety of others by adhering to WHS policies and procedures.
