Role Overview:
We are seeking a dedicated and compassionate Aged Care Case Manager to join our team. In this autonomous role, you will play a crucial part in assessing needs, developing and delivering home care plans to clients and their carers.
About the Role:
This is a fantastic opportunity to work with a leading provider of high-quality aged care services. As an Aged Care Case Manager, you will be responsible for conducting initial assessments to determine client eligibility and primary needs, undertaking comprehensive assessments and creating care plans, providing information and advice to clients about self-management and coordination of their care plans, and developing individual budgets within the available funding to meet their needs and goals.

Required Skills and Qualifications:
To be successful in this role, you will need a tertiary qualification in Health or a related field and evidence of relevant case management experience. You will also require strong communication skills, with the ability to communicate effectively and compassionately with people from diverse backgrounds, problem-solving ability, and a solutions-focused approach.

About You:
- Tertiary qualification in Health or a related field and evidence of relevant case management experience
- Experience in assessments and developing care plans
- Experience in setting and management of consumer budgets
- Great communication skills with the ability to communicate effectively and compassionately with people from diverse backgrounds
- Problem solving ability and a solutions focused approach
- An understanding of Aged Care Reforms and a passion for the industry
- Familiarity with community aged care services and referral pathways
- Exceptional customer service skills
- Ability to work autonomously; sound time management skills and the ability to handle multiple priorities simultaneously
- Preparedness to travel to regional Victoria and Northern NSW as required
