About the RoleThe Access Mental Health Professional delivers comprehensive assessments, assertive outreach, and triage to individuals and families to ensure access to effective treatments. The role includes acute care interventions, supporting clients recently discharged from inpatient services, and responding to urgent care needs.
Key Responsibilities- Deliver comprehensive assessments for individuals presenting with acute mental health needs
- Provide assertive outreach and acute care interventions
- Support clients recently discharged from inpatient services
- Respond to urgent care presentations with appropriate treatment options
- Collaborate with clients, families, and carers to develop treatment plans
- Coordinate with inpatient units and community-based services to ensure continuity of care
- Relevant tertiary qualifications as a Registered Nurse or allied health professional, including completion of a new graduate program or equivalent 12 months of experience, and current registration with AHPRA
- Experience in assessment and management of people with acute mental health disorders
- Strong negotiation, verbal, and written communication skills
- Knowledge of recovery-oriented care and collaboration with consumers and their families
- Computer competency and familiarity with IT systems used in health settings
