About the Role

We are seeking a skilled and compassionate Lifestyle Facilitator to join our team at OneLife NQ Inc. as a Disability Support Worker. As a key member of our team, you will play a vital role in supporting individuals with disabilities to live their best lives.

Key Responsibilities

  • Assist clients with daily living activities, including personal care and social support.
  • Help clients achieve their personal goals and objectives.
  • Provide support in areas such as daily living, social support, and personal care.
  • Work collaboratively with clients, families, and other stakeholders to develop and implement individualized support plans.
  • Contribute to a positive and inclusive team environment.

Requirements

  • Certificate III or IV in Disability, Individual Support (or equivalent) is highly desirable.
  • Personal experience working with individuals with disabilities is highly regarded.
  • Exceptional communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Sound knowledge of the NDIS practice standards.
  • Current First Aid and CPR certificates.
  • Current NDIS Worker Screening (yellow card) and Working with Children Check (blue card).
  • Be vaccinated for Covid-19 (at least 2 doses).
  • Completed free NDIS orientation modules.
