We are seeking a dedicated and compassionate individual to join our team as a Lifestyle Facilitator (Disability Support Worker) in Mackay.
As a Lifestyle Facilitator, you will play a vital role in supporting our clients to live their best and most independent lives. Your responsibilities will include assisting with daily living tasks, providing social support, and promoting personal growth and development.
Key Responsibilities:
- Assist clients with daily living tasks, such as showering and toileting
- Provide social support and companionship to clients
- Help clients achieve their personal goals and objectives
- Maintain accurate records and documentation
- Work collaboratively with the team to ensure high-quality care and support
Requirements:
- Certificate III or IV in Disability, Individual Support (desirable)
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility with rostering and availability
- Current First Aid and CPR certificates
- NDIS Worker Screening (yellow card) and Working with Children Check (blue card)
- Sound knowledge of the NDIS practice standards
