We are seeking a motivated Aboriginal Health Pathways Worker to join the Pathways to Good Health Team. This multidisciplinary health service supports children and young people in out-of-home care, providing tailored health management plans.

The Aboriginal Health Pathways Worker will assist children, young people, and their families in navigating health services and overcoming barriers to access. The role involves liaising with VACCA and local First Nations healthcare providers.

This is a 0.4 EFT fixed-term position until June 2027, working across our Werribee site with VACCA, and at Caroline Springs and Melton campuses of Western Health.

Key responsibilities:

  1. Identify and address barriers to healthcare access for children and young people.
  2. Coordinate with external services such as dental, maternal and child health, and child protection services.
  3. Participate in multidisciplinary assessments for First Nations children and young people, providing expert input.
  4. Make referrals based on assessment outcomes and support access to these services.

To be successful in this role, you should:

  1. Have knowledge of and experience with the local Aboriginal community and culture.
  2. Work effectively and sensitively, understanding the holistic needs of clients and families.
  3. Respect the rights of children and young people to informed choice and independence.
  4. Maintain confidentiality, privacy, and effective communication.
  5. Be self-motivated, able to work independently, and eager to contribute to the team.
