We are looking for a motivated Aboriginal Health Pathways Worker to join the Pathways to Good Health Team. Pathways to Good Health is a multidisciplinary health care service for children and young people who have entered out of home care. The service aims to provide these children and young people with health management plans that meet their unique needs in a timely manner.
As the Aboriginal Health Pathways Worker, you will be responsible for assisting children/young people and their families to navigate the service and break down barriers to involvement with the service. You will also be responsible for liaising and relationship building with VACCA and local First Nations Health care providers.
This is a .4 EFT fixed term role until June 2027. This position works across our Werribee site with VACCA and Caroline Springs and Melton campus of Western Health.
Key responsibilities:
- Identify and navigate barriers to children and young people accessing health care.
- Liaise with multiple external services while providing service navigation. For example, including: dental services, maternal child health nurses and child protection.
- Attend the multidisciplinary assessment for First Nations children and young people providing your expertise.
- Make referrals for the children and young people in care based on the needs identified in the multidisciplinary assessment and provide support to accessing these services.
To be Successful in this Role you:
- Have knowledge and familiarisation of the local Aboriginal culture and community.
- Demonstrate the ability to work effectively and sensitively with an understanding of the holistic needs of clients and their families.
- Respect for and understanding of the child/young person's right to informed choice, independence and determination of services required and delivered.
- Ability to apply principles of confidentiality, privacy and maintain confident communication.
- Can work independently, be self-motivated and want to bring your knowledge base to the wider team.
