About the Role:
  • To facilitate meaningful connections between staff, patients, and community organizations, fostering a collaborative environment that enhances patient care and social support services.
  • We are seeking an individual who can develop and maintain strong linkages with hospital wards, departments, and community services, ensuring seamless admission, discharge planning, and referral processes.
  • The successful candidate will network and liaise effectively between Aboriginal and Torres Strait Islander community organizations, public sector agencies, patients, their families, and programs within our Hospital and Health Service.
  • Promoting the identification process for Aboriginal and Torres Strait Islander patients among staff during admission and optimizing related processes are key responsibilities.
Key Skills and Qualifications:
  • Strong understanding of cultural competency and its application in healthcare settings.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Able to work independently and as part of a team, prioritizing tasks effectively.
