About the RoleThis role will see you working as part of our multidisciplinary team to deliver early intervention, crisis management, and aftercare services to clients referred to the Social Health Culture Care Connect program.
You will be responsible for assessing and supporting the needs of individuals and families, operating within a strengths-based framework that prioritises client goals and outcomes.
As a Social Health Care Coordinator, you will provide case management and crisis support, delivering a range of assessed social and wellbeing supports to meet the holistic recovery needs of your clients.
Key Responsibilities- Manage intake referrals and provide high-quality support to clients.
- Maintain accurate and up-to-date data records using LogiQC quality management systems.
- Respond appropriately to critical incidents, providing wellbeing first aid and emergency support to clients in distress.
- Collaborate with internal stakeholders to facilitate efficient and effective service delivery.
- Liaise with external agencies and organisations to promote ATSICHS Brisbane services and enhance partnerships.
- Contribute to team efforts through self-management and collaborative practice.

Benefits- An annual salary ranging from $74,351 to $82,964, depending on experience and qualifications.
