Social Health Care Coordinator Specialist

The Role:
We are seeking an experienced Social Health Care Coordinator to join our team, providing early intervention, crisis management, and aftercare services to clients referred to the Social Health Culture Care Connect program. As a key member of our team, you will assess and support the needs of families and individuals, delivering case management and crisis support that meets their holistic recovery needs.
Duties:
- Intake referrals and provide high-quality support to clients.
- Data management and accurate recording of client information.
- Responding to critical incidents and providing wellbeing first aid to clients in distress.
- Maintaining a high level of quality management through the use of LogiQC quality management systems.
- Providing integration and assisted referrals and coordination between ATSICHS services to facilitate efficient, responsive, and integrated mental health and social needs service delivery.
- Liaising with community organisations and government agencies for information gathering, referral, networking, and promoting ATSICHS Brisbane Services.
- Self-management and contributing to team collaboration.
- Cultivating productive working relationships at all levels, fostering internal and external relationships, facilitating cooperation, trust, and respect, valuing differences and diversity, and guiding, mentoring, and focusing on staff development.
Salary and Benefits:
As a valued member of our team, you will receive an annual salary of $74,351 to $82,964 per annum, depending on your experience and qualifications. You will also enjoy 11.5% Super + Not-for-Profit Salary Packaging, which increases your take-home pay.
Requirements:
- Tertiary qualification in Social Work or Human Services or related field, or demonstrated experience in providing support and case coordination to people with complex needs.
- Demonstrated ability to apply high-level practice judgement when working with clients to set and achieve goals.
- Ability to work with culturally and socially diverse groups of clients and apply a culturally responsive framework to guide interventions with the support of staff and management.
- Ability to work with diverse people and presentations, providing case-managed social work-driven supports.
- Demonstrated ability to work with minimal direction and effectively communicate with and work collaboratively as part of a multi-disciplinary team, contributing to the development of creative and innovative services.
- Demonstrated ability to record accurate and consistent case notes.
- Demonstrated IT capability, including experience in computer-based data management systems and Microsoft Office.
- A willingness to travel across the ATSICHS Brisbane region of services and work across different sites.
- A willingness to provide social work supports and engagement to patients outside of a clinic environment.
- Working with Children's Card (Blue Card) and NDIS Worker Screening (Yellow Card) or eligibility to apply.
Preferred Requirements:
- Previous experience in a multi-disciplinary team environment.
- Previous experience working in an Aboriginal Medical Service.
