We are a not-for-profit community-owned health and human services organisation delivering on the unique health and wellbeing needs of Aboriginal and Torres Strait Islander people in greater Brisbane and Logan.

The Role

This role will assess and support the needs of families and individuals by operating within a strengths and priority-based framework.

As a Social Health Care Coordinator, you will deliver case management and crisis support for clients, providing a range of assessed social and wellbeing supports to clients in meeting their holistic recovery needs.

Key Responsibilities

  • Intake referrals and provide quality support
  • Data management and analysis
  • Responding to critical incidents and providing wellbeing first aid
  • Quality management and continuous improvement
  • Integration and coordination of services
  • Liaising with community organisations and government agencies
  • Self-management and team contribution

Requirements

  • Tertiary qualification in Social Work or Human Services or related field
  • Demonstrated ability to apply a high level of practice judgement when working with clients
  • Ability to work with a culturally and socially diverse group of clients
  • Ability to work effectively as part of a multi-disciplinary team
  • A willingness to travel across the ATSICHS Brisbane region of services

Preferred

  • Previous experience in a multi-disciplinary team environment
  • Previous experience working in an Aboriginal Medical Service
