Job Title: Senior Care Manager

The role of the Senior Care Manager is to provide high-quality care and support services to clients in their own homes. This involves developing and implementing individualized care plans that meet the unique needs and goals of each client.

Key Responsibilities:

  • Conduct comprehensive assessments of client needs and develop person-centred care plans that focus on fostering independence, wellness, and reablement.
  • Work collaboratively with clients, their families, and other stakeholders to understand how they can use their Home Care Package funding to achieve positive outcomes that optimize their health, wellbeing, and safety.
  • Build and maintain effective partnerships with a range of stakeholders, including clinical experts, allied health professionals, and home support workers, to ensure clients receive the right care and support to meet their individual needs.
  • Travel may be required to visit clients in their homes, and a company vehicle will be provided.

Requirements:

  • Experience in the health and/or aged care sector, with a strong understanding of person-centred care principles and demonstrated skills in networking and developing strong links with local service providers, suppliers, and referrers.
  • Attention to detail, a positive and practical attitude, and strong time management skills.
  • A current driver's license is also required.
  • Diploma level qualifications in health/aged care/disability/social welfare (preferred) will be highly regarded.
