Job Title: Team Leader Case Management Services

As a seasoned and dedicated Team Leader, you will play a pivotal role in overseeing the Home Care Package Program. Collaborating closely with senior management ensures seamless delivery of services that meet high standards.

Key Responsibilities:
  • Coordinate and lead the Home Care Package Program to guarantee effective service provision.
  • Monitor client outcomes and assess service efficacy through the case management model of care.
  • Manage package expenditure prudently to ensure services are delivered within budget guidelines.
  • Contribute to marketing strategies to enhance growth in Home Care Packages.
  • Identify and implement improvements to optimize care quality and competitiveness.
  • Foster and maintain relationships with key stakeholders, including government agencies, healthcare providers, and community organizations.
  • Ensure compliance with data collection requirements and funding targets.
  • Effectively manage compliments, complaints, and feedback to drive continuous improvement.
Requirements:
  • A minimum of 3 years' experience in aged care, home care, or a related field.
  • Sound knowledge of the Home Care and/or Aged Care industry, encompassing the Aged Care Quality Code of Conduct, Aged Care Standards, and accreditation processes.
  • Strong communication skills with the ability to build rapport with internal and external stakeholders.
  • Experience in budgeting and fund management associated with Home Care Packages.
  • Willingness to travel both intra- and interstate.
  • Availability for after-hours on-call duties.
