Job Title: Outreach Navigator Specialist

The Outreach Navigator plays a critical role in the Coordinated Intake program, providing housing stabilization assistance and services to individuals and families experiencing homelessness. This position requires daily engagement with unsheltered individuals, collaboration with various teams, and accurate documentation of supportive services for housing stability.

Required Skills:

  • A Bachelor's degree in social, behavioral, or human services;
  • At least 2 years of professional experience in case management, community outreach, and/or the social service field;
  • Personal experience with homelessness or housing instability is highly valued.

Benefits:

As an Outreach Navigator, you will have the opportunity to work collaboratively with enrolled participants under the Home for Every Neighbor initiative led by the City of Cleveland Project Management Team.

Others:

The ideal candidate will be able to maintain a caseload ratio of 1:20, assisting participants with determining housing eligibility, program enrollment, and housing coordination through public resources provided through the Home for Every Neighbor initiative. This is a collaborative role requiring teamwork and coordination amongst established community partners, such as Outreach, Housing Case Management, and Unit Acquisition teams.
