
Empowering Young Lives: A Community Care Coordinator Role

We are seeking a dedicated professional to fill the role of Community Care Coordinator. This position involves supporting First Nations children aged 0–9 with disabilities or developmental delays, empowering their independence, community inclusion, and overall quality of life.
- You will be responsible for building strong, respectful relationships with First Nations families, ensuring their voices and preferences are at the forefront.
- Observations of a child in their natural environment (face to face and/or remotely via video conference, where applicable) will be undertaken to assess their functioning and support needs.
- Supporting First Nations families to establish meaningful goals & aspirations for their child and family and track progression toward goals.
- Providing community connections and linkages to mainstream services in the local area to improve children's access to the right supports and meaningful social participation.
- Experience working with First Nations children and families is highly desirable.
- Understanding of child development (0–9 years) is essential.
- Knowledge and connection to local First Nations communities is also required.
- Confidence in building community networks and service pathways is vital.
- Ability to promote a culturally safe and supportive work environment is crucial.
- Tertiary qualification in Early Childhood or Allied Health is desirable.
- Understanding of child protection principles and Keep Them Safe is mandatory.
- Valid Driver's Licence (P1 minimum) and willingness to travel across Northern NSW, including Grafton, Lismore, and Tweed Heads.
- WWCC & NDIS Worker Screening Check.
