About Supreme Community Care

We are seeking an experienced and dedicated Support Worker to join our team. The ideal candidate will have a passion for delivering high-quality care and support to individuals with various disabilities, including autism, motor neuron disease, stoma care, spinal cord injuries, intellectual disabilities, and mental health.

The Role

As a Support Worker, you will be responsible for providing personal care, social and community access, and assisting with daily activities while ensuring that everything you do is focused on achieving the participants' goals. This may include conducting tasks such as hoist use and manual handling.

Key Responsibilities

  • Provide personal care and assist with daily activities
  • Facilitate social and community access
  • Assist with hoist use and manual handling
  • Work with participants with various disabilities, including autism, motor neuron disease, stoma care, spinal cord injuries, intellectual disabilities, mental health, and others

Requirements

  • Proven experience in the industry
  • Completed or working towards a Certificate III or higher in Individual Support - Disability or equivalent
  • A people-focused approach to working with adults, children, and families in a community-based role
  • Flexibility, reliability, and a commitment to providing exceptional service
  • Working with Children Check (Blue Card)
  • Working with Disability Check (Yellow Card/ NDIS Screening Card)
  • Current driver license and access to a reliable registered and comprehensively insured vehicle
