Care Coordinator Position

We are seeking a Care Coordinator to join our team at St Catherines Aged Care in Bathurst. This role involves providing essential lifestyle, social and wellbeing services for our residents.
The successful candidate will work closely with our residents to understand their needs and preferences, and provide individualized support to ensure they feel valued and respected.
- Encourage resident participation in activities and events
- Assist in the planning and delivery of group and individual lifestyle services
- Implement innovative programs and activities for residents and the wider community
- Provide direct care during activities such as bus trips
- Maintain a cooperative team spirit
To be successful in this role, you will need:
- A Certificate III or Cert IV in Leisure and Health or Individual Support
- Strong communication skills and a positive attitude
- Able to establish professional relationships with residents and their families
- A reliable team player
