Lead Care Coordinator

Job Description:
As a Registered Nurse, you play a vital role in coordinating the care of each patient. This includes incorporating nursing values of knowledge, theory and practice to assess, plan, implement and evaluate patient needs and those of significant others.
Key Responsibilities:- Coordinate patient care for comprehensive support and well-being
- Collaborate with interdisciplinary teams for optimal patient outcomes
- Develop and implement care plans tailored to individual patient needs
- Maintain accurate and detailed patient records
Strong leadership, coaching skills, excellent interpersonal skills, good judgment and decision-making skills are essential. You must also work efficiently as part of a team and maintain confidentiality.
Requirements:- Diploma in Nursing or Bachelor Degree in Nursing
- Active registration with Nurses Association
- Minimum 2 years of specialized nursing experience in family practice, surgical, palliative care or oncology
- Knowledge of Extra-Mural Services and community services available
- Valid New Brunswick Class 5 Driver's License and good driving record
- Current CPR Level C certification
