Aged Care Service Coordinator

About the Role

The Aged Care Service Coordinator role supports our organization's mission to strengthen person-centred primary health care.

This position develops and coordinates activities related to older people's health, aged care, and palliative care.

Dual Focus
  1. Servicing the strategic objectives of a network for Western NSW Primary Health
  2. Covering key areas of aged care and palliative care support

The successful candidate will have strong project management skills and experience working in healthcare. They will be able to coordinate with various stakeholders, develop reports and provide excellent customer service.

The ideal candidate will have knowledge of the Australian Government's policies on ageing and age-related issues. Experience working with older adults, their families and carers is essential.

Aged care services coordination requires strong communication skills, both written and verbal. The ability to work autonomously and as part of a team is also required.

Requirements

To be successful in this role, you will need:

  • A tertiary qualification in a related field (e.g., nursing, social work, or business)
  • Significant experience in aged care or a related field
  • Strong project management skills
  • Excellent communication and interpersonal skills
  • Able to work independently and as part of a team
