Job Description

We are seeking a skilled professional to join our team as a Nurse in the community. The successful candidate will provide care for patients who require tube feeding in their own homes, building strong relationships with carers and healthcare professionals along the way.

This role involves working with adult patients in the Sunderland and Tyne and Wear area. To be considered for this position, you will need to be a Registered Nurse (RGN, RSCN or RNLD) with at least 12 months of experience and living within or close to the area.

You will also require a full UK Driving License, which we will cover with a company car, fuel card, and lunch allowance when you're on the job.

Key Responsibilities:

  • To develop and deliver comprehensive training to patients, carers, and healthcare professionals to support safe discharges from the hospital and ongoing quality care at home.
  • To complete regular patient reviews and be available to assist with questions, issues, and troubleshooting requests.
  • To help prevent unnecessary hospital stays, including tube changes within the community.
  • To play a pivotal role in nurturing patient and healthcare professional relationships.

  • The ability to work independently while effectively managing your diary.
  • A clear understanding of the UK Healthcare Market, including the NHS.
  • Excellent communication skills with internal and external stakeholders, including patients and healthcare professionals.
