We are seeking a skilled Nurse to join our team.

About the Role

The successful candidate will provide care for patients who require tube feeding in their own homes. You will work closely with carers, healthcare professionals, and other stakeholders to deliver high-quality patient care.

This role involves working with adult patients within the Sunderland and Tyne and Wear area. To be considered for this position, you must have at least 12 months' experience as a Registered Nurse (RGN, RSCN or RNLD) and live within or close to the area. A full UK Driving License is also required.

Key Responsibilities:

  • To develop and deliver comprehensive training to patients, carers, and healthcare professionals to support safe discharges from hospital and ongoing quality care at home.
  • Complete regular patient reviews and be available to address questions, issues, and troubleshooting requests.
  • Assist in preventing unnecessary hospital stays, including tube changes within the community.

Requirements:

  • Proven ability to work independently while effectively managing your diary.
  • Clear understanding of the UK Healthcare Market and NHS policies.
  • Excellent communication skills with internal and external stakeholders, including patients and healthcare professionals.
