Community Care Worker Job

We are looking for Community Care Workers to provide support services in client homes, assisting with daily living activities and promoting choice and independence.

Key Responsibilities:
  • Promote client choice and positive routines in accordance with our service delivery model.
  • Work shifts in set time blocks/pattern rostering.
  • Drive client and company vehicles as required.
  • Support clients proactively to enhance their life choices at home and in the community.
  • Report changes in clients' health, care plans, or if additional services are needed.
  • Stay updated on policies, procedures, and activities via email, intranet, and other methods.
  • Ensure own health and safety and that of others in the workplace.
  • Protect client rights and confidentiality, maintaining dignity and privacy during care.
Requirements:
  • Completion of a minimum of Certificate III in Individualised Support, Home & Community Services, Aged Care, or equivalent.
  • Current full driver's license.
  • Current Level 2 First Aid Certificate & CPR.
  • Reliable motor vehicle with valid registration and comprehensive third-party insurance.
  • Smart mobile device.
  • Willingness to obtain NDIS Clearance if not already held.
  • Police Clearance within the last 6 months or willingness to obtain one.
  • Access to the internet and experience with computers and applications.
  • Genuine desire to serve the community and work in a mission-driven organisation.
