Independent Living Support Specialist

Empower older people to live independently in their own homes as a Care Facilitator with BaptistCare. If you have a passion for supporting individuals and families, this role could be perfect for you.
As a dedicated Care Facilitator, your day will involve home visits, developing individualised care plans, participating in service development and evaluation, building relationships with families and carers, and implementing best practice approaches for care.
- You will require a Bachelor's Degree or Diploma in Community Services (Service Coordination) or equivalent knowledge and experience.
- A valid driver's license is also necessary.
- You should possess intermediate level computer skills.
- Excellent time management and organisational skills are crucial.
