Job Description\

We are seeking a skilled professional to join our team as a Case Manager. The Case Manager will provide support to adults with intellectual disabilities/autism and their families, serving as an advocate and ensuring choice and satisfaction with services. The ideal candidate will have excellent interpersonal and communication skills, be proficient in speaking, reading, and writing English, and have knowledge of the system of care in Maine.

The Case Manager will work closely with a multi-disciplinary team to identify needs and develop a plan of care, initiate referrals and monitor services, while coordinating and advocating for services identified in the plan. This is a hybrid remote position after successful onboarding.
