Case Manager - Transitional Housing

We are seeking a skilled Case Manager to join our team in providing support services to homeless and formerly homeless young adults. This role will involve working closely with clients to develop individualized service plans, promoting their housing stability and independence.

About the Role
  • Develop and implement service plans to help clients maintain housing and achieve personal goals, including mental health, substance use management, income increase, and self-sufficiency.
  • Conduct home visits and follow-ups to support housing stability and independence.
  • Promote a culture of diversity, equity, and inclusion through anti-racist practices.
Key Qualifications
  • Bachelor's degree in social work, counseling, or a related field.
  • Lived experience of homelessness or housing instability, especially among BIPOC, LGBTQIA+, and young adults.
  • Experience in providing services to the target population and/or case management preferred.
  • Valid driver's license with less than six points and proof of automobile insurance.
Job Benefits

This is a non-exempt position with a starting wage of $20.00/hour. The work hours are Monday through Friday, 8:30 am – 5:00 pm.

The ideal candidate will possess strong communication and interpersonal skills, as well as the ability to work effectively with diverse populations.
