Job Opportunity: Case Manager

The primary objective of the Case Manager role is to deliver case management services to homeless and formerly homeless young adults aged 18-24 under the supervision of the Young Adult & Rapid Rehousing department.

This involves creating and implementing personalized service plans to support clients in maintaining housing and achieving personal goals, thereby fostering their independence and self-sufficiency.

Main Responsibilities:
  • Crafting and executing tailored service strategies to promote housing stability and meet individual client needs.
  • Conducting regular recertifications, home visits, and follow-ups to ensure clients receive necessary support for sustained housing and progress towards independence.
  • Fostering an inclusive environment through anti-racist practices across the department and agency.

Requirements:
  • Bachelor's degree in a relevant field such as social work or counseling.

  • Previous experience in providing services to target demographics and/or case management is advantageous.
