Job Opportunity:

Clinical Aged Care Assessors play a pivotal role in identifying and meeting the complex needs of clients. They refer them to suitable aged care services as per the Aged Care Act.

  • Conducting thorough assessments to determine client requirements and eligibility for home support services
  • Collaborating with clients, families, and multidisciplinary teams as necessary
  • Working collaboratively with colleagues to foster a supportive team environment
  • Maintaining currency of knowledge relating to aged care services
  • Assisting with continuous improvement frameworks to evaluate and enhance assessment processes and outcomes

Essential Qualifications and Skills:

  • Tertiary qualification in a health-related discipline directly related to Health, Aged Care, or a specialist area
  • Current unrestricted registration with the relevant Australian regulatory agency
  • Australian Driver's license and access to a registered, insured vehicle
  • Minimum one year experience delivering services in Aged Care settings and/or to Aged persons, including those living with dementia
  • Willingness to apply for a Police Check
