Senior Aged Care Specialist

We are seeking an experienced Senior Aged Care Specialist to join our dynamic team.

The ideal candidate will have a strong background in aged care and be able to provide high-quality assessments and support to clients.

  • Key Responsibilities:
  • Collaborate with clients, families, and multidisciplinary teams as required.
  • Conduct accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services.
  • Review support plans to meet client's changing support needs.
  • Work effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes.
  • Maintain currency of knowledge relating to aged care services both funded and non-funded.
  • Assist with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes.
Requirements:
  • Tertiary qualification – in a health-related discipline directly related to Health, Aged Care or related specialist area.
  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA).
  • Australian Driver's license and access to a registered, insured vehicle.
  • Minimum of one year demonstrated experience in Australia or Overseas directly delivering services in Aged Care settings and/or to Aged persons including to people living with dementia.
