Clinical Aged Care Role

We're seeking a skilled Clinical Aged Care professional to make a meaningful impact in the lives of older Australians.

This exciting opportunity will allow you to utilize your expertise to assess client needs and ensure they receive the appropriate care and support.

  • You'll conduct thorough assessments using the Integrated Assessment Tool (IAT) to identify and meet complex client needs, referring to relevant aged care services as necessary under the Aged Care Act.

Key responsibilities include:

  • Collaborating with clients, families, and multidisciplinary teams as required to provide seamless care coordination;
  • Working collaboratively with colleagues to foster a positive and supportive team culture;
  • Conducting accurate and comprehensive aged care assessments using the IAT;
  • Reviewing and updating support plans to address changing client needs;
  • Maintaining currency of knowledge related to aged care services and best practices;

To succeed in this role, you'll require:

  • A tertiary qualification in a health-related discipline directly related to Health, Aged Care, or a specialist area;
  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA);
  • An Australian Driver's license and access to a registered, insured vehicle;
  • A minimum of one year demonstrated experience in Australia or overseas delivering aged care services, preferably in dementia-specific settings;
  • A willingness to apply for a Police Check;
