Social Support and Respite Worker

Sunbury and Cobaw Community Health is seeking a skilled and compassionate Support Worker to join our Social Support Groups and Respite team. As a Support Worker, you will play a vital role in supporting individuals to reduce social isolation and build connections with others.
Key Responsibilities- Provide support and companionship to individuals in a group setting, either in the community or at one of our sites.
- Offer 1:1 support to individuals in the community, working with clients referred via My Aged Care or the National Disability Insurance Scheme (NDIS).
- Build rapport with clients and their carers, utilizing excellent communication, planning, and problem-solving skills.
- Work collaboratively with the team to provide a comprehensive support service.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility to work across multiple sites, including Sunbury and Kyneton.
- Ability to maintain confidentiality and handle sensitive information.
- Child-friendly hours with flexibility available.
- Professional supervision and support.
