Job Description:

We are seeking a dedicated and passionate individual to join our team as an Intensive Support Program Case Manager. This is a unique opportunity to make a real difference in the lives of vulnerable young people and their families.

The successful candidate will work collaboratively with key partners, internal and external stakeholders, and other SHS services to provide intensive case management to young people aged 16-24 who are homeless or at risk of homelessness. The position requires strong communication and interpersonal skills, with the ability to build rapport and engage with clients from diverse backgrounds.

The ideal candidate will have tertiary qualifications in Social Work, Social Science, Community Welfare or equivalent, with a minimum of 2 years' experience working with vulnerable young people and families. A strong understanding of strengths based, client centered and trauma informed practice is essential, as well as experience in all aspects of case management, case plans and evaluations.

The role involves providing direct financial assistance via brokerage to young people, supporting them to live independently and achieve access to stable, longer term housing options. The Case Manager will also be responsible for developing and maintaining positive relationships with key partners and stakeholders, ensuring that young people's needs are accurately understood and met.

Required Skills and Qualifications:

  • Tertiary qualifications in Social Work, Social Science, Community Welfare or equivalent
  • Minimum 2 years' experience working with vulnerable young people and families
  • Strong understanding of strengths based, client centered and trauma informed practice
  • Experience in all aspects of case management, case plans and evaluations
  • Highly skilled in communication and interpersonal skills
