Job Opportunity: Intensive Support Case Manager

About the Job
">The Intensive Support Program provides transitional accommodation, case management, and access to community and specialist support services for young people aged 16-24 who are homeless or at risk of homelessness.
Our team delivers strengths-based, flexible, trauma-informed, and client-centered case management while supporting young people to build their capacity and self-determination.
Responsibilities include:
">- Working within a culturally appropriate framework that focuses on individual strengths
- Undertaking case management responsibilities including reviews, reporting, and administrative tasks
- Supporting young people to develop their skills and independence
- Recording and assessing referrals and connecting with relevant services
- Acting as an advocate for young people and ensuring their needs are accurately understood and met
- Providing direct financial assistance via brokerage to young people
- Maintaining positive professional relationships with stakeholders
- Ensuring accurate recording of all statutory and program-related information requirements
Requirements
">Applicants should possess:
">- Diploma/Bachelor qualifications or higher in Social Work, Social Science, Community Welfare, Youth Work, or a related field
- A current NSW Working With Children Check and National Police Check
- Minimum 2+ years of experience working with young people aged 16-24, preferably in roles involving homelessness support
